IN-HOME VETERINARY CARE
Jewell Animal Hospital is now offering in-home veterinary care visits for both existing and new clients!
Can’t make it to our office? No problem! We can come to you and your pet!
Does your pet need a physical exam, vaccinations, or medications? We can take care of these and other services in your home.
We can also collect blood or cytology samples and bring them back to the hospital. Your veterinarian will call you back with the results later on.
If any medication is prescribed as part of the home visit, we’ll deliver it to you within 24 hours of the visit.
As you may have guessed, we don’t perform in-home surgeries, x-rays, or ultrasounds.
** In-home visits can be scheduled Monday – Friday, 9 AM – 5 PM.
**The base cost for an in-home pet appointment is $150. Regular exam charges are not included in this fee.
New clients will be required to complete client and patient paperwork before the first in-home visit. You can print the forms here, fill them out and scan them. Then email the forms to us at firstname.lastname@example.org or fax them to 773.271.1901. Download Form

At the end of the in-home visit, the veterinary technician or veterinarian will call the clinic to invoice the charges they discussed with you. Your payment will be processed over the phone at this time.
An invoice can be emailed to you upon request.
We accept Visa, MasterCard, Discover, and American Express. We will accept personal checks with your driver's license as proof of identification.

Please keep in mind:
- A two-hour block of time is required
- Scheduling is dependent on the veterinarian, time, and Zipcar availability
- A $150 non-refundable deposit is required. This will be applied to the visit charge as long as the appointment is kept.
PLEASE NOTE: The veterinarian and veterinary technician will leave our clinic at the time of your appointment. So, if your appointment is at 9 AM, the veterinarian and technician will leave our office at 9 AM. One of our customer service representatives will call your contact number on file to tell you their departure time and expected arrival time.
